Research Interests
I currently serve as the Director of Sinus and Allergy, Department of Otolaryngology-Head and Neck Surgery and also as a Research Associate in the Center for Microbial Pathogenesis at the Research Institute, Nationwide Children's Hospital. The focus of our research mission is to develop personalized diagnostic and treatment regimens for the care of chronic rhinosinusitis (CRS). This will allow us to deliver personalized medicine to our patients and reduce the number of antibiotics prescribed for the diagnosis of sinusitis or the common cold, one of the 8 major goals of the Obama Workgroup on Healthcare Reform and a major named goal of Health People 2010. Specifically, it is now known that many patients with CRS have a greater concentration of non-typeable Haemophilus influenzae (NTHI) in the bacterial biofilms located within their sinus cavities compared to healthy patients. The specific hypothesis being tested in our research is that the presence of NTHI biofilms, which are known to be highly recalcitrant, are positively correlated with the development of CRS and that unique lipooligosaccharides and inflammatory byproducts obtained from nasopharyngeal lavage fluids and/or swabs of sinus secretions may be used as a non-invasive biomarker for CRS. These areas can be non-invasively swabbed and/or lavaged using rigid sinonasal endoscopy in the office setting. As a result, patients with symptoms of CRS specifically associated with NTHI biofilms could possibly obtain a non-invasive test in the physician’s office that would allow the clinician to make a more accurate diagnosis and objectively follow each patient’s responsiveness to customized therapy. Patients without evidence for a chronic bacterial infection, or evidence for a non-infectious cause of their chronic sinus disease maybe treated conservatively without antibiotic therapy, and instead with treatments specifically tailored to their disease. Achieving these goals would allow us to deliver more efficient care to our patients and improve quality while decreasing health care expenditures from often inappropriate and unnecessary empiric treatments.
