Research Interests

As an Otolaryngologist subspecializing in the practice of Otology, Neurotology, and Cranial Base Surgery, I am committed to improving the lives of people inflicted with hearing and balance disorders.

My research at OSU has focused on the pathogenesis and treatment of vestibular schwannomas (VS), intracranial, skull base neoplasms originating on the vestibulocochlear nerve. I have published two clinical retrospective reviews on patients treated surgically at OSU, looking at tumor nerve of origin, CSF leak rates, hearing preservation rates, facial nerve outcomes, etc. An IRB application is currently in preparation to collect this data prospectively. I have also published several case reports regarding related and unrelated skull base pathologies. My basic science work has focused on VS. In 2007/2008, I determined that the AKT intracellular signaling pathway is active in human vestibular schwannomas and that this pathway helps drive VS growth/proliferation (Torok Award, 2007 - Podium presentation @ The American Neurotology Society Meeting; published Otology & Neurotology 2008). Along with my collaborators, we then demonstrated that inhibiting the AKT pathway can suppress tumor growth both in cell culture and in animal models (this work was published in EJC 2009, the highest impact publication ever coming from the OSU NF2/VS research group). In August of 2009, I received an NIH/NIDCD K08 grant (5-years; $1.16 million) as Principal Investigator) to study AKT/OSU-03012 in greater detail. The Children's Tumor Foundation, the primary group leading the research effort against NF2-associated tumors such as VS, has provided me with a drug development grant (2007) and invited me to give a podium presentation regarding VS drug development efforts at OSU during their national meeting in 2008. In late 2007, I was invited to participate in an international consensus panel - establishing the goals and outcome measures for clinical trials targeting NF2-associated tumors.

Because other growth promoting pathways may also be relevant to pathogenesis (and therefore treatment) of vestibular schwannomas, I have recently helped establish a cross-discipline collaboration (Dr. CS Chen - medicinal chemistry/pharmacology, Dr. Kenneth Chan - expert in pharmacokinetics/pharmacodynamics at the OSU Comprehensive Cancer Center, and Dr. Matthew Ringel - Endochrinology/Thyroid Cancer) to look closely at the PAK signaling pathway in several human cancers and in NF2-associated tumors wuch as schwannomas. We have developed 18 novel small molecule inhibitors of the PAK kinase and are initiating biological screening assays to determine their potency and their specificity. In the future, lead agents will be identified and tested both in vitro and in vivo for efficacy, toxicity, etc.

A new initiative this year has been the James Care Hearing Conservation Program. Patients undergoing chemotherapy – particularly the platinum agents - lose substantial amounts of hearing due to treatment, and unfortunately, many are not adequately aware of chemotherapy-associated ototoxicity prior to initiating therapy. At The Ohio State University Medical Center, most of these patients are not screened for hearing loss before chemotherapy, and typically, they are not monitored for ongoing hearing loss during/after treatment. This compromises the informed consent process and creates significant long term, potentially preventable morbidity for cancer survivors. The immediate goals of The James-Care Hearing Conservation Program (JCHCP) are to standardize the hearing screening process at Ohio State for all patients undergoing conventional chemotherapy, provide these patients and their treating physicians with real-time data about drug-induced ototoxicity, and to establish a prospective database of chemotherapy-related hearing outcomes for future research. In the long term, the JCHCP may integrate into the existing Phase I/Phase II clinical trials program at The OSU Comprehensive Cancer Center – providing OSU CCC investigators with the means of assessing ototoxicity during administration of investigational drugs. In addition, this program can be used to determine the feasibility, safety, and efficacy of novel intratympanic (IT) treatments for chemotherapy-associated hearing loss as they become available.
